To compete in sports, students will still need a sports physical on the middle school form which you can find below and also on the PWCS Student Activities website.  We will not offer on site physicals for the 2021-2022 school year.  Parents/Guardians should print the form and take with them to physical appointment. 

SPRING sport pages have been updated.  Please remember athletes will need a current physical and proof of concussion training to participate.  Also, we would like to remind all parents/guardians that only athletes and coaches are allowed at try-outs and practices.  We ask that parents pick up there students at 4:50 by the modular/trailer in the parking lot.  Activity buses will be available for try-outs and Tuesdays, Wednesdays, and Thursdays after that.  Please let me know if you have any questions.

COVID-19 Screening Form 

At the current moment, this form is not needed.  If anything changes I will let the athletes know.
